Ask your HMO whether the selected RB-ABC branch and proposed service are accredited under your plan.
Request a Letter of Authorization (LOA), if your provider requires one. Confirm its validity and any co-payment.
Bring your HMO card, a valid ID and the approved LOA or referral, when required.
PhilHealth provides an Animal Bite Treatment Package through accredited package providers. Eligibility, covered services and documents should be confirmed with the treating facility.
Verify that your chosen center is an accredited Animal Bite Treatment Package provider.
Bring your PhilHealth ID or PIN, a valid ID and your Member Data Record, if available or requested.
The branch can explain eligibility, claim documents, covered services and any remaining charges.
Seek prompt in-person assessment at an animal-bite center. Online forms, HMO approvals and benefit checks should not delay care.
